for ovarian cancer testing. We will continue to post all raw data, the exact methods, and the results of the blinded trial(s) for public scrutiny. These are very important times for biomarker discovery and validation. In keeping with the National Cancer Institute Director’s 2015 challenge, effective biomarkers for early detection, risk stratification, and monitoring can save lives and reduce suffering.

The global need to define normality: the 99th percentile value of cardiac troponin.

BACKGROUND How to select a presumably normal population for the establishment of 99th percentile cutoffs for cardiac troponin assays has not been adequately addressed. Lack of attention to this question can result in misleading medical decision cutoffs. CONTENT From our review of the peer-reviewed literature, including international recommendations, no uniform procedure is followed and no uni...

Impact on Patient Management and Outcome of Switching between 2 Contemporary Sensitive Cardiac Troponin Assays.

BACKGROUND Myocardial infarction is characterized by an increase of cardiac troponin I (cTnI) above the 99th percentile of a reference population. Our hospital switched from 1 contemporary cTnI assay to another and observed a doubling of cTnI results above the assays' respective 99th percentile cutoffs. We investigated the potential impact on inpatient management and outcomes. METHODS We perf...

High-Sensitivity Troponin Assays: Boon or Bane for the Cardiologist?

The Troponin Era The advent of the “troponin era” has its roots in the redefi nition of acute myocardial infarction (AMI) by the joint committee of European and American cardiologists in 2000.1 Troponin was identifi ed as the preferred serologic biomarker for acute coronary syndrome (ACS). Since then, troponin assays have continued to evolve with improved sensitivity and reproducibility, leadin...

Plasma 99th percentile reference limits for cardiac troponin and creatine kinase MB mass for use with European Society of Cardiology/American College of Cardiology consensus recommendations.

BACKGROUND The European Society of Cardiology/American College of Cardiology (ESC/ACC) consensus document for definition of myocardial infarction (MI) is predicated on increased cardiac troponin or creatine kinase (CK) MB mass above the 99th percentile reference limit. The purpose of this study was to determine the plasma (heparin) 99th percentile reference limits for the leading in vitro diagn...

99th percentile and analytical imprecision of troponin and creatine kinase-MB mass assays: an objective platform for comparison of assay performance.

Cardiac troponins in blood are the most sensitive and specific biochemical markers of myocardial damage and are paramount for classification, risk stratification, and customized therapy in patients with acute coronary syndromes (1, 2). Despite the overt advantages, some important obstacles to troponin analysis and interpretation have remained, such as assay standardization, interference, preana...
